Before setting up an appointment, we will verify your specific insurance benefits and discuss them with you. 

All patients, including those who are in-network, are responsible for their deductible if they have one. Once the deductible is met, in-network patients are then responsible for their co-pay or co-insurance and cannot opt to self-pay. Out-of-network patients can either work with us to have their insurance billed or can request to opt-out and self-pay.

Patients who are not covered by insurance can pay out of pocket. Our income-based fees are determined by a combination of adjusted income and financial circumstances.
